Acupuncture & Sports Injuries and Pain

Treating Sports Injuries and Pain
The Tendino-Muscle Meridians

Treating the tendino-muscle meridians (TMM) is one of the first things to consider in treating sports injuries and pain. Their treatment is often overlooked and under-utilized.
The activation of these important pathways can be crucial to effectively treating patients with acute trauma or repetitive-stress injuries.

Let's start with a description of the TMM (jin mai, jin jing). They are described as a channel network that circulates qi over the superficial aspect of the body.1 This includes the skin, the muscles, and the tendons. They travel in the "depressions and planes between muscles and tendons."2 Furthermore, the muscle meridians are comprised of wei qi ("protective" qi), which is not "contained" within a vessel. Thus, their pathways have a bit more freedom and flexibility. I suggest that the reader view the tendino-muscles meridians as a general pathway of qi in the muscles, fascia and tendons. They are responsible for many functional activities of the musculoskeletal system, and are therefore involved in many sprains, strains and other traumatic injuries.

The jing-well point is the only acupuncture point directly shared by both the primary meridian and the tendino-muscle meridian. After the jing-well point, the TMM follows the path of the primary meridian past the first, second and third joints of the extremity (i.e., the ankle or wrist, the knee or elbow, the hip or shoulder). Because the wei qi of the meridian is not contained within a vessel, its pathway is superficial, broad and diffuse.

Most texts describe the pathology of the TMM as syndromes of either excess (shi) or deficiency (xu). As you can guess, an excess syndrome of the TMM produces pain. It is usually described as diffuse and distending, and frequently found at multiple ah shi points in the affected muscle, fascia or tendinous areas. Most sources emphasize that pain is elicited by light palpation and pressure. Remember, the TMMs are superficial and the qi is not contained within a vessel. Thus the pain will not manifest as fixed, localized and deep. Other accompanying symptoms include stiffness, swelling, spasm and contraction. You may observe inflammatory signs of a yang nature, including heat and redness.

A typical clinical picture is the downhill skier who has been on the slopes all day long. They complain of pain, tightness and a feeling of fullness in the quadriceps muscle group; a typical ache after a good hard workout. Upon palpation, there are numerous ah shi points on the anterior thigh, elicited using relatively light pressure. These painful points are found in the superficial layers of the muscle, along the Stomach channel. This may be diagnosed as an acute excess condition of the tendino-muscle meridian of the Stomach. Simply bleeding St 45 (Li Dui), the jing-well point, should improve the condition significantly, possibly without any further treatment.

If, however, our ambitious skier had taken a hard fall, the quadriceps could have sustained a slight strain or tear in the muscle tissue. This case would present quite differently. Palpation would reveal fixed pain at the site of the tear, elicited with deep pressure. While the TMM may also be involved, the primary lesion is in deeper tissues of the muscle, and jing-well point treatment would not be sufficient to fully heal this case. You would undoubtedly need additional points and techniques in the treatment protocol for the patient.

Bleeding is the preferred way to treat a jing-well point. Using sterile lancets and surgical gloves, swiftly needle-prick the point while holding firm pressure on the finger or toe. Try to get 10 large drops of blood to drain from the point. If the quantity of blood is not sufficient, a simple trick is to lower the patient's foot or arm off the table. This will often get the necessary drops of blood.

Jing-well points are treated on the same side as the site of injury. The yang tendino-muscle meridians are usually more responsive and clinically effective than those of the yin meridians. Treatment may include more than one jing-well point when the site of injury extends to multiple meridian pathways. Care should be taken in bleeding yin meridian jing-well points, considering all of their contra-indications.

In sports medicine acupuncture, I generally organize treatment into a systematic approach using four steps. Activation of the TMM is one of the techniques of step one. With all the techniques of this initial step, the practitioner is looking for an immediate effect on the patient. Carefully observe if there is a decrease in pain or an increase in range of motion. My experience is that up to 80 percent of patients with pain due to injury or trauma will have some degree of improvement from bleeding the indicated jing-well point(s). Patients with qi stagnation and blood stasis in the deeper tissues - conditions like a sprained ankle, shoulder tendonitis, or joint pain - will often experience a 15 to 25 percent improvement.

I recently treated a runner from a local high-school track team. He had a slight tear of the quadriceps tendon at its attachment to the ilium at the anterior superior iliac spine. Manual muscle testing of both hip flexion and knee extension reproduced pain and weakness. As this injury is likely on the TMM of the Stomach, the jing-well point St 45 (Li Dui) was treated with a bleeding technique. Other local and distal points on the Stomach meridian were also used.

Afterward, I immediately retested hip flexion and knee extension. These tests revealed both a strong muscle and significantly reduced pain at the tendinous attachment. This was the first acupuncture treatment for this young athlete, and he was amazed to see such an immediate change. Within several treatments and a week or so of rest, he was back to full training and competition.

In conclusion, don't underestimate how much improvement can be achieved by the simple technique of bleeding one or more jing-well points. Because it is so important in treating sports injuries and pain, the jing-well point is often the first technique I use to start a treatment.

Acupuncture therapy for stroke-caused conditions such as paralysis, speech and swallowing problems, and depression is commonly used in the Orient. In China and Japan, an acupuncturist is likely to start therapy as soon as possible after a stroke. However, my recommendation is a delay of 2 weeks before acupuncture for strokes caused by bleeding in the brain, 10 to 20% of strokes (instead of the usual blood vessel block or clot). The wait is due to studies showing that acupuncture opens blood vessels for better flow and decreases clotting and inflammation. These effects are desired early when arteries are blocked, but after a stroke caused by bleeding has occurred, it is safer to wait until the bleeding blood vessel has clotted and is unlikely to bleed further before providing the acupuncture.

Acupuncture is done on a daily basis in China. Uncertain as to whether daily acupuncture is essential, even in Western countries, some acupuncturists with experience in treating stroke with acupuncture believe treatment 3 times a week is optimal. Several different approaches have been used to treat stroke, demonstrating that acupuncture for this disorder remains a healthcare art: Traditional Chinese Yang meridian point therapy, Chinese scalp acupuncture,

Dr. Yamamoto's YNSA Japanese scalp therapy, Korean Koryo Chim hand acupuncture, supplemental ear (auricular) acupuncture, and Xingnao KaiQiao (a newer therapy by Professor Shi Xuemin) are each advocated by a number of treatment centers in Oriental countries. One need not know in depth the approach of each, but it is important to know that more than one approach is available and used.

Does acupuncture really work to help stroke victims improve? Many studies involving thousands of patients have been published in China and Japan, and 2 of 3 studies from Scandinavia, demonstrated significant help. These studies indicate that patients get well faster, perform better in self-care, require less nursing and rehabilitation therapy, and use less healthcare dollars. However, since most studies come from China, they get little credence from the Western medical community because researchers in China do not appear to be published unless their results are highly positive, so publication bias is possible. And, no money has been made available in the United States for studies needed to confirm the claims of experts in China and Japan of indeed helping stroke patients. Such studies, if done well, demand significant funding; sources of such money are difficult to find. Most physicians, including rehabilitation experts, have appeared unwilling to consider acupuncture therapy, not due to bias but because the published studies do not necessarily meet research study criteria for the United States.

Acupuncture is a safe therapy, and my experience has taught that this is especially true when helping stroke victims. Even discomfort is generally minimal. If one compares the possible positive help to be obtained with the risks associated with acupuncture, my contention is that acupuncture is worthy to attempt for stroke therapy. Studies are needed in the United States to prove this to medical skeptics. It will be important for such studies to involve skilled acupuncturists with experience in stroke therapy.

Adding acupuncture to rehabilitation therapy obviously increases the cost; daily-to-3 times-weekly treatment is needed for 2-4 weeks or longer. Concern for added cost would perhaps disappear if the end result demonstrates more self-care and less dependence on family and health providers.

HOW MANY TREATMENTS WILL I NEED?

The number of treatments needed differs from person to person. For complex or long-standing conditions, one or two treatments a week for several months may be recommended. For acute problems, usually fewer visits are required, and for health

ARE THERE ANY SIDE EFFECTS TO THE TREATMENT?

Usually not. As energy is redirected in the body, internal chemicals and hormones are stimulated and healing begins to takeplace. Occasionally the original symptoms worsen for a few days, or other general changes in appetite, sleep, bowel or urination patterns, or emotional state may be triggered. These should not cause concern, as they are simply indications that the acupuncture is starting to work. It is quite common with the first one or two treatments to have a sensation of deep relaxation or even mild disorientation immediately following the treatment. These pass within a short time, and never require anything more than a bit of rest to overcome.



WHAT ARE THE NEEDLES LIKE? DO THEY HURT?



People experience acupuncture needling differently. Most patients feel only minimal pain as the needles are inserted; some feel no pain at all. Once the needles are in place, there is no pain felt. Acupuncture needles are very thin and solid and are made from stainless steel. The point is smooth (not hollow with cuttingedges like a hypodermic needle) and insertion through the skin is not as painful as injections or blood sampling. The risk of bruising and skin irritation is less than when using a hollow needle.

Because your doctor carefully sterilizes the needles using the same techniques as for surgical instruments, or uses disposable needles, there is no risk of infection from the treatments.



DOES ACUPUNCTURE REALLY WORK?

Yes. In the past 2,000 years, more people have been successfully treated with acupuncture than with all other health modalities combined. Today acupuncture is practiced widely in Asia, the Soviet Union, and in Europe. It is now being used more and more in America by patients and physicians. Acupuncture treatments can be given at the same time other techniques are being used, such as conventional Western medicine,osteopathic or chiropractic adjustments, and homeopathic ornaturopathic prescriptions. It is important that your physician-acupuncturist know everything that you are doing, so he or she can help you get the most benefit from all your treatments.



DO I HAVE TO BELIEVE IN ACUPUNCTURE FOR IT TO WORK?

No. Acupuncture is used successfully on cats, dogs, horses and other animals. These animal patients do not understand or believein the process that helps them get better. A positive attitude toward wellness may reinforce the effects of the treatment received, just as a negative attitude may hinder the effects of acupuncture or any other treatment. A neutral attitude ("I don't know if I really believe in this.") will not block the treatment results.



ARE THERE ANY "DO'S AND DONT'S" FOR ME ON THE DAY OF A TREATMENT?

Yes. To enhance the value of a treatment, the following guidelines are important:

* Do not eat an unusually large meal immediately before or after your treatment.
* Do not over-exercise, engage in sexual activity, or consume alcoholic beverages within 6 hours before or after the treatment.
* Plan your activities so that after the treatment you can get some rest, or at least not have to be working at top performance. This is especially important for the first few visits.
* Continue to take any prescription medicines as directed by your regular doctor. Substance abuse (drugs and alcohol) especially in the week prior to treatment, will seriously interfere with the effectiveness of acupuncture reatments.
* Remember to keep good mental or written notes of what your response is to the treatment. This is important for your doctor to know so that the follow-up treatments can be designed to best help you and your problem

I have two very interesting case of Piles or Haemorrhoid:

Case 1 : Lady Mrs A Age 45 years.

A lady visited my clinic in 1990 with throbbing pain in the rectum. She has been suffering this type of problem for the last 3 years on and off with chronic constipation. She passed the stool mixed with blood. The pain of rectum internally and externally and when the tumour burns, are moist, protrude considerably and can scarcely by replaced, with violent shooting pain in the back and feeling of constriction.

She came to me four time regularly where I used to prescribe’ Nux Vom, Sulp’ alternately and ending with ‘Thuja’ and this lady was completely cure the piles she is suffering for 3 years. Till now no more complaint.

Case No. 2: Mrs K, A Nurse from local Hospital, Age 35 years.

She visited me May 2007 with burning and swelling of the anus. On first visit, I asked her to sit down, she refused because she can’t sit down due to ‘painful and swelling of anus’, She even can’t walk normally due to the ‘violent shooting pain’.
On inspection, I have seen ‘a tomour’ about 4 cm diameters with ‘cherry like’ red color at the tip of the anus.

After discussion with the patient I decided to use ‘Integrated Medicine’, externally I use 4 Acupuncture points. One Ac Pt of Ren 1 and Du 1 and two Ac Pt of “Whezong’ and ‘She-hen-kong’. For 30 minutes without stimulation and orally I have given ‘Bell 200c’ one dose before acupuncture therapy and one dose of Nux Vom 200c after acupuncture.
I ask her to come back next morning and when she arrived next morning, she came will big smiles. Surprisingly that within only ‘one therapy’ her piles has reduced to 50 % and she has no more pain.

On the second visit next day, I have repeat the same procedure and ask to come back on the 3rd day.

On the 3rd visit, she was completely cured!
On July 2007 this young lady was posted to Saudi Arabia and no more news from her up till now, I assume that she is completely healthy and happy.

Mr A , Age 16 was refferd by E.N.T. Specialist of S.M.S. Medical College and Hospital, Jaipur, India. He was first admitted in the Hospital on 30th March 1981 and was operated.

The Operation Note is:,

D/L done under G.A and biopy taken. Diagnosed as Multiple Papilloma of local cords.
I was consulted on 1st April 1981 and the symptoms was hoarseness only. The was no modality. On my previous experience, I prescribed Calc Carb 1000 one dose through he had no symptom of the medicine

12 April 1981 : No improvement. Calc Carb 1000 one dose
21 April Voice slightly clear. No medicine
2 May: No further improvement, Thuja 1M one dose
1 June : Papilloma disappeared as reported by ENT specialist of SMS Hospital
17 June : The patient reported that the more he excerts the more clear is the voice. Rhus Tox 1M one dose.

Since then there is no report.

WASHINGTON – Secretary of State Hillary Rodham Clinton said Wednesday that plans by a small church in Florida to burn the Muslim holy book are "outrageous" and "aberrational" and do not represent America.

In remarks to the Council on Foreign Relations, Clinton lamented that the tiny Dove World Outreach Center congregation in Gainesville had gotten so much attention for what she called a "distrustful and disgraceful" means of marking the ninth anniversary of the Sept. 11, 2001, terrorist attacks.

"It is regrettable that a pastor in Gainesville, Florida, with a church of no more than 50 people can make this outrageous and distrustful, disgraceful plan and get the world's attention, but that's the world we live in right now," Clinton said. "It is unfortunate, it is not who we are," she said.

Despite pleas from senior Obama administration officials that it will put Americans abroad at risk, church Pastor Terry Jones vowed to go ahead with the event.

Clinton appealed for Jones to reconsider and cancel. And, in the event he goes ahead with the plan, she suggested to laughter from the audience, that the news media ignore it.

"We are hoping that the pastor decides not to do this," she said. "We're hoping against hope that if he does, it won't be covered as an act of patriotism."

"We want to be judged by who we are as a nation, not by something that is so aberrational and we will make that case as strongly as possible."

Rawatan jenis ini boleh dikatakan diguna oleh semua masyarakat kuno yang bertamadun awal seperti orang-orang Bunti Afrika, Arab Tigris, Mesir Kuno, India dan China. Dikalangan ahli-ahli perubatan, mereka memberi penghurmatan kepada China, kerana pengamalan jenis ini banyak diamalkan oleh rakyat jelata sejak 4000 tahun yang silam sampai kehari ini.



Sejarah juga membtuktikan bahawa bangsa China merupakan suku bangsa pertama didunia yang mencatit sejarah tentang Perubatan Akupuntur dengan lengkap dan tersusun yang ditulis oleh Maharaja China yang dinamakan sebagai ‘ Huang Ti The Yellow Emperor Canon of Medicine’ yang dikarang pada tahun 2000 sebelum Masehi (Terjemahan buku ini ada dalam simpanan Prof Dr Nik Omar yang didapatinya dari Beijing, China)



Pada zaman kuno orang-orang China menggunakan batu-batu tajam, kayu, buluh dan lain-lain sebagai asas perawatan untuk menekan atau menusuk dibahagian tertentu badan. Dizaman moden ini, alat-alat tersebut telah diganti dengan menggunakan jarum-jarum halus yang telah tersedia sucihama atau sterile, yang diperbuat daripada berbagai logam seperti ‘Jarum Silver, Copper dan Jarum Emas’

Diantara banyak jarum, Jarum Emas adalah yang termahal dan dikira sebagai jarum yang dapat mengeluarkan tenaga ‘qi’ yang 100 kali ganda kekuatan dari jarum silver. ( Jaum Emas ada dalam simpan Prof Dr Nik Omar yang didapatinya dari Negeri China )



Pada jarum-jarum ini sudah tersedia ada magnetik yang kita namakan sebagai kuasa ubat pada jarum tersebut. Apakah tusukan jarum akupunktur menyakitkan



Kebiasaannya tusukan jarum akupuntur dimana-mana bahagian badan tidak mendatangkan apa-apa kesakitan kerana jarum-jarum yang digunakan adalah terlalu halus, lembut dan tajam. Jarum tersebut diletakkan dibahagian lapisan atas kulit sahaja diantara 1 ‘cun’ hingga 2 ‘cun’ atau 0.5cm hingga 1 cm sahaja. Kalau pun sakit, ianya bagai digigit semut pada tusukan pertama sahaja.


Apakah jarum akupunktur bahaya



Jarum akupuntur moden telah datang dalam keadaan siap dalam packing yang telah disuci-hama atau sterile. Setiap pengamal akupuntur sudah pasti pegang jarum tersebut terlebih dahulu sebelum menusujkan kepada badan pesakit. Oleh itu jika ada bakteria atau hama tentu doktor dahulu yang dijangkitinya . Hal ini sama sekali tidak diingini oleh setiap doktor.

Anda tidak perlu khuatir setiap jarum akupuntur adalah bersih dan selamat.



Apakah jarum akupunktur merebak Aids



Belum pernah ada sejarah bahawa AIDS merenbak melalui jarum akupuntur.

AIDS hanya merebak melalui jarum yang tidak bersih, yang diguna pakai oleh penagih dadah, mereka bergilir-gilir menggunakan jarum yang sama dan menyuntik lebih dari berpuloh-puloh kali jarum yang sama, tanpa bersih, tanpa rebus atau tanpa di masukkan kedalam autoclave.



Apakah beza antara jarum akupunktur dengan jarum biasa



Jarum biasa mempunyai ukuran badan yang besar dan keras. Didalammnya berongga untuk mengalirkan ubat-ubat sewaktu melakukan sesuatu suntikan. Suntikan dengan jarum-jarum ini mendatangkan kesakitan 80 - 90 %

Sementara Jarum Akupuntur, halus sebesar rambut yang diperbuat daripada logam khas bermagnet, tidak ada lubang, tidak berkarat dan tidak boleh patah. Sakit pada suntikan jarum akupuntur hanya 5 % sahaja.

Bagi orang-orang yang takut pada jarum atau orang-orang sensitif, kesakitan boleh mencapai 20 % Boleh dikatakan 80 % pesakit menyatakan tidak mengalami apa-apa kesakitan.

Mengikut kes rekod dari Dr Tang Chunyu ,Anshan Central Hospital, China didapati sebanyak 50 kes perenial allergic rhinitis (Resdung) telah dibawa ke hospital ini untuk mendapatkan rawatan secara akupunktur telinga.

Dr Xiao Jifang dari Gangxi College of TCM, China telah membuat kajian terhadap 112 orang pesakit buasir yang telah mengalami kesakitan didubur setelah melakukan pembedahan buasir. 61 kes lelaki sementara 51 kes wanita. setelah melkaukan beberap terapi dengan kaedah akupunktur dengan menggunakan titik-titik B2 (Zanzhu), Hegu (LI4) dan Tangli (H5)
